Evidence-based research to support self-management programs was provided in the first module. A range of self-management support programs for patients is available, some of which are adaptations of the original Stanford Chronic Disease Self-management Program (CDSMP). Generic and disease-specific self-management programs are on offer in Australia and New Zealand.(1)

  • The Personal Helpers and Mentors (PhaMS) Program (Australia)

    The Personal Helpers and Mentors Program is a new program that supports people whose lives are seriously affected by a severe mental illness. It focuses on recovery – demonstrating that people with mental illness can lead a fulfilled life in the community with the same opportunities as other people. People who join the program will work with their own Personal Helper and Mentor who will support them on their recovery journey.
  • Stanford Internet Chronic Disease Self-management Program
    Each workshop runs for six weeks, with new lessons each week. Participants are asked to log on at their convenience two or three times each week for a total of one to two hours. There is no requirement that participants log in at the same time. Each participant in the workshop receives a copy of the companion book, Living a Healthy Life With Chronic Conditions, 3rd Edition.

  • Arthritis Self-management Courses (ASM) Australia
    Good programs currently exist for arthritis sufferers for their chronic condition self-management. These are offered by a variety of organisations.

  • Panic Center
    Panic Center offers a 16-week panic and anxiety program. After the completion of an initial test, a letter on the reported symptoms is provided for the health professional supporting a collaborative approach to treatment. This site also offers an online chat forum for support and a medication glossary (once signed in).

  • CRUfAD – Clinical Research Unit for Anxiety and Depression
    CRUfAD Clinic is a series of web-based courses to teach patients about better managing their health.

Select a patient that you currently work with.

Record a list of the knowledge and skills that this patient requires.

From the information provided in this module, identify some resources and agencies which could be helpful for the patient (and their carer where appropriate) and make a list on the notepad provided.

Take time to investigate the resources that you have identified and jot down some notes that give reasons why the resource is useful for your selected patient.
